What Will My Complimentary Consultation Include?
You will receive a one-on-one consultation with our Clinic Director to determine whether you are a candidate for treatment. During this consultation you may also have questions answered about your specific injury or about how an integrated treatment plan can benefit you.
How Should I Prepare for My Appointment?
Please complete new patient forms in advance prior to arriving for your appointment. Along with a copy of your driver’s license to the online New Patient forms above, personal injury patients will require some additional items. See the link above for Personal Injury Forms. If you have been previously treated for a specific injury or condition you will want to obtain your medical records to share with our doctors. Recent X-Rays, MRI’s or other diagnostic reports may expedite the diagnostic process.
How Long Will My Appointment Take?
Your initial visit will last about one hour.
